Welcome to ReJEN Mobile Health. These Terms of Service ("Terms") explain the rules for using our website, rejenmobilehealth.com, and for booking and receiving our services. ReJEN Mobile Health is a d/b/a of [Legal Entity Name, LLC] ("ReJEN," "we," "us," or "our"). By using our website or booking a service, you agree to these Terms. If you don't agree, please don't use the site or book services.

1. Who we are and what we offer

ReJEN is a mobile health service based in Anthem, Arizona. We bring care to clients in Anthem, North Phoenix, Cave Creek, Carefree, Scottsdale, Peoria, and Glendale. Our services include IV hydration and nutrient therapy, medically supervised GLP-1 weight management programs, and peptide therapy.

Treatments are administered by licensed registered nurses (RNs) and, in some cases, a nurse practitioner (NP). All treatments are reviewed and approved by our supervising medical provider, a licensed nurse practitioner.

2. The website is not medical advice

Content on our website is for general education only. It is not medical advice, diagnosis, or treatment, and reading it does not create a provider-patient relationship. A provider-patient relationship begins only after you complete our intake process and a medical evaluation with one of our licensed providers. Please see our Medical Disclaimer for more.

If you are experiencing a medical emergency, call 911. Our services are not for emergencies.

3. Eligibility and medical screening

Every service requires a medical evaluation before treatment.

  • IV therapy: A Good Faith Exam (GFE) is completed in person before your first treatment and as required after that.
  • GLP-1 and peptide programs, and other patient-specific prescriptions: Your evaluation is completed by telehealth with our nurse practitioner.

You agree to provide complete, accurate, and current health information, including medications, allergies, and medical history. Our providers use their independent clinical judgment. They may decline, delay, modify, or stop any treatment if they believe it is not safe or appropriate for you. Booking an appointment does not guarantee that you will receive treatment.

4. Minors

Clients ages 10 to 17 may receive IV therapy only with a parent or legal guardian present for the entire visit and a signed consent form on file. GLP-1 and peptide programs are available to adults 18 and older only, unless our provider determines otherwise. We do not provide services to children under 10.

5. Telehealth

Some evaluations and follow-ups happen by telehealth. To receive telehealth services from us, you must be physically located in Arizona at the time of your visit. Telehealth has limits. Your provider may require an in-person visit if they decide it is needed for your safety. You are responsible for having a private location and a reliable connection for your visit.

6. Prescription medications

If our provider prescribes a medication, it is filled by a licensed third-party pharmacy and, in most cases, shipped directly from the pharmacy to the address you provide. For GLP-1 programs, a nurse will schedule an in-person teaching visit after your medication arrives.

Some medications we prescribe, including compounded medications, are not approved by the U.S. Food and Drug Administration (FDA). Compounded medications are prepared by a licensed pharmacy for an individual patient and have not been reviewed by the FDA for safety, effectiveness, or quality. Your provider will discuss the risks, benefits, and alternatives with you before prescribing.

You agree to:

  • Use prescribed medications only as directed, and only for yourself
  • Store medications as instructed
  • Tell us right away about side effects, new medications, or changes in your health
  • Attend required follow-up visits

Self-administered medications. Some peptides and medications may be self-administered after a nurse teaches you how. You agree to follow those instructions and to contact us with any questions before administering.

7. Mobile visits

We come to your home, office, or another location within our service area. You agree to provide a safe, clean, well-lit space with a place to sit or recline, and to keep pets secured during your visit. Our nurses may decline or end a visit if they feel the environment is unsafe. We do not charge travel fees within our service area.

8. Appointments, payment, and refunds

Appointments are currently booked by phone or text at (614) 599-9843. No deposit is required to book. Payment is due at the time of service and is processed securely through Square. We accept cash, major credit and debit cards, and HSA/FSA cards. We do not accept insurance and do not bill insurance companies. You are responsible for confirming whether a service is an eligible HSA/FSA expense.

Prices on our website may change without notice. The price you pay is the price confirmed at booking.

Cancellations, no-shows, and refunds are governed by our Refund & Cancellation Policy, which is part of these Terms.

9. Communications

By booking with us, you agree that we may contact you by phone, text, and email about your appointments, care, and account. Marketing emails are sent only if you opt in, and you can unsubscribe at any time using the link in any email. We do not send marketing texts. If that changes, we will ask for your separate written consent first. Message and data rates may apply.

10. Acceptable use of our website

You agree not to misuse our website. This includes trying to access areas you aren't authorized to, interfering with the site's operation, submitting false information, or using automated tools to copy content.

11. Intellectual property

All website content, including text, graphics, logos, and images, belongs to ReJEN or its licensors. You may view and print pages for personal, non-commercial use. You may not copy, modify, or distribute our content without written permission.

12. Third-party services and links

We use trusted third-party services to run our practice, including IntakeQ for intake forms, consents, and records, Square for payments, and licensed pharmacy partners. Those services have their own terms and privacy policies, and we are not responsible for their content or practices.

13. Assumption of risk

All medical treatments carry some risk. IV therapy risks include bruising, discomfort, infiltration, infection, vein irritation, and allergic reaction. GLP-1 medications and peptides carry their own risks, which your provider will review with you. You will be asked to sign an informed consent form before treatment. By receiving treatment, you confirm that you understand and accept these risks.

14. Disclaimer of warranties

Our website is provided "as is" and "as available." To the fullest extent allowed by law, we make no warranties about the website, and we do not guarantee any particular result from any treatment. Individual results vary.

15. Limitation of liability

To the fullest extent allowed by law, ReJEN and its owners, providers, and staff are not liable for indirect, incidental, special, or consequential damages arising from your use of our website. Our total liability for any claim related to the website or our non-clinical services will not exceed the amount you paid us in the 12 months before the claim. Nothing in these Terms limits liability that cannot be limited under Arizona law, including liability for professional negligence.

16. Indemnification

You agree to hold ReJEN harmless from claims that result from your breach of these Terms, your misuse of the website, or your failure to provide accurate health information or follow care instructions.

17. Governing law

These Terms are governed by the laws of the State of Arizona. Any dispute will be handled in the state or federal courts located in Maricopa County, Arizona.

18. Changes to these Terms

We may update these Terms from time to time. The "Last updated" date at the top shows when they last changed. Continuing to use our website or services after an update means you accept the new Terms.
